This comprehensive evaluation is included for active members of the concierge practice. Non-members may access it as a one-time package, which includes the initial assessment and one follow-up visit only. Ongoing care, communication, or treatment outside of these visits is not available without membership. Laboratory testing is not included in the package. If labs are recommended, they may be billed to your insurance by the lab company. Participation in this evaluation does not establish an ongoing doctor-patient relationship unless membership is initiated.
