šFull Job Description:
Interviews patients, assess priorities, and documents biopsychosocial assessments and progress notes.
Provides patients and families with education on advance directives and end-of-life care.
Consults with members of the multi-disciplinary team in liaison with community resources to support the patientās care.
Assists patients/families with obtaining community resources.
Develops and maintains working relationships with community social agencies.
Performs other duties as assigned.
