Their conversation covers what most patients are never told before augmentation surgery.
Dr. Gonzalez's clinical finding from 1,500+ explant cases: every breast implant patient has an ongoing immunological response and chronic inflammation, every day. Not just patients who feel sick. All of them. Many have normalized their fatigue, joint pain, or brain fog without connecting it to their implants.
He also found capsular contracture in more than 80% of his explant patients, far higher than what physical exams detect. The retropectoral placement technique that was supposed to reduce contracture simply hid it from examination.
The clinical standard both surgeons apply: total capsulectomy in every explant case, without exception. En bloc removal is a cancer operation and appropriate only when malignancy is confirmed.
On alternatives to implants: native tissue reconstruction restores shape, projection, and cleavage without a device. In Dr. Gonzalez's data, 94% of patients did not request fat transfer after reconstruction. Their own tissue was enough.
