Acute painful unilateral red eye with some blurring of vision in a young male with back pain and stiffness is suggestive of acute anterior uveitis. All these symptoms suggests presence of a gene called HLA-B 27 which runs in the family but affects some and while others are spared. The constellation of back pain, rib pain , red eye, HLA-B 27 positivity along with sacroilitis on MRI or Radiograph are suggestive of Ankylosing Spondylitis(AS). Tell your ophthalmologist that you are HLA-B27 positive when you meet him for red eye. Biologicals, the medicines which are used in AS can also help in uveitis prevention.
