Subject: Safety and Roadway Violations – Dan Swazenson, Inc. near Dingmans Bridge
To Whom It May Concern,
I observed employees of Dan Swazenson, Inc. performing work near Dingmans Bridge in New Jersey. During this time, I noted several serious safety and compliance violations:
1. No PPE in use – The worker on site did not wear basic required personal protective equipment (PPE), including a hard hat, while working on or near the roadway. This is an OSHA violation.
2. Unsafe travel in a Scorpion truck – An employee was riding on the back of a Scorpion truck while it was in motion and actively pulling cones. This is a roadway safety violation. Employees must be seated inside the vehicle and wearing a seatbelt when the truck is moving.
3. Failure to comply with roadway safety rules – The lack of a hard hat and proper protective equipment while placing or retrieving traffic control devices exposes the worker and others to unnecessary risk.
These issues represent both OSHA safety violations and traffic law violations. Immediate corrective action is needed to ensure worker safety, public safety, and compliance with regulations.
