Ultrasound Guided Barbotage is an established technique for the treatment of calcific tendonosis of the rotator cuff in the shoulder. It has been shown to be much more effective than other treatments with over 70% of improvement of pain.
With technological improvements modern ultrasound machines allows a skilled operator to 'see' the calcification and assess it in 3 dimensions. It allows the surgeon to place a needle directly into the calcium bundle and break it up.
Using the ultrasound image, the needle is guided into the calcification under direct vision. The calcification will then be attempted to be aspirated/withdrawn. To encourage healing the calcification is punctured several times under local anaesthetic. Finally the tendon is bathed in more anaesthetic to allow for a more comfortable procedure.
