Second toe pain. Too often patients will come into my office with long term pain in the 2nd toe or the ball of the foot by the 2nd or third toe. Unfortunately they are typically misdiagnosed with a mortons neuroma . There are some very accurate exams to differentiate between a neuroma and a capsule tear. As can be seen in the MRI above there is a partial tear of the ligament of the 2nd toe. The patient was treated elsewhere with multiple injections thinking it was a neuroma, but it was really a plantar plate tear of the 2nd toe. It was unfortunate not only because of the pain that the patient suffered but the steroid injections made the capsule weaken more to the point that it tore. A simple MRI could have helped make the proper diagnosis and get the patient on the way to recovery. Dr Wrotslavsky has extensive experience treating pain in the ball of the foot. Most people can get better with non surgical therapies. However there are cases where surgery is necessary.
